Clearing Tournament Eligibility Affidavits

2006

The District will be clearing “Tournament Eligibility Affidavits” on June 26, June 27 and June 28 at

Pied Piper Pizza Listed below is a time for each league to arrive.

Prepare to stay, be patient and above all, make sure your paper work is Complete.

Each League needs to bring the following (“Affidavit” will not be processed unless everything is completed):

1) Tournament Eligibility Affidavit: Make sure all pages are filled out, and bring two (2) sets with the proper signatures.

2) Proof of Residency: Three (3) Separate pieces of Proof of Residency are needed for each player.

Make copy of each “Proof of Residency” on a separate piece of paper.

3) Original Birth Certificate, certified Birth Certificate, Statement in Lieu of, or Passport: Bring the originals.

No Photo Copies

4) Medical Release: Have Parents fill out completely.

5) Map of Boundaries: Needed for each Division that shows where each player lives, and written (north, south, east, and west) Boundaries. The player will be listed in the same order as on the “Tournament Eligibility Affidavit”. Each map must have signature of the President and Mike Ray, District Administrator, and must be dated this year. If a player is on Waiver II(d) or IV(h), show where player originally lived in your boundaries, and where the player lives currently. Bring two (2) maps.

6) Regulation II(d) and Iv(h) Waiver Claim Forms already approved by DA: Must have three pieces of Proof of Residency of where the player originally lived in your boundaries, and where the player lives currently.

7) 2006 Team Rosters: Needed to verify players team.
